Medicare reimbursement policy changes constantly, and nobody publishes how much. We track every change across all 7 A/B MAC contractors — who between them administer the 12 A/B MAC jurisdictions — plus CMS national policy. Each change is linked to the originating CMS or MAC document. This page is that record, stated plainly, including what it cannot yet tell you.

The complete-coverage window
Rates and comparisons on this page are drawn only from the window beginning June 1, 2026 — the date from which our record is complete and prospective. In that window we have recorded 146 policy changes over 9 weeks, an average of 16.2 per week.
We are not yet publishing a contractor-by-contractor comparison.
The complete-coverage window holds 146 policy changes; 150 are required before MAC contractors are compared. A comparison drawn from our pre-2026-06-01 archive would reflect how far back each source still publishes its history, not how much policy actually changed.
We could publish one today by counting our full archive. It would be wrong, and we would rather be useful than early. The comparison publishes automatically once the complete-coverage window passes 150 recorded changes.
Methodology, and what this data cannot tell you
We monitor CMS, the Federal Register, and all 16 MAC jurisdictions — the 12 A/B jurisdictions and the 4 DME jurisdictions. Every tracked change links to the originating public document, so any figure here can be checked against the source rather than taken on our word.
Our record begins June 1, 2026. We hold older material, collected when we first indexed each source, but we do not compute rates or comparisons from it — and neither should anyone else. Sources retire old documents at different rates, so an archive reaches further back for some contractors than others. Counting it would measure how much history each source still exposes, not how much policy actually changed. That distinction produces a compelling, and false, upward trend. We decline to publish it.
420 tracked changes carry no published date in their source document. They are counted in the corpus total and excluded from every rate. We do not infer a date we were not given.
This series therefore starts narrow and strengthens each quarter as the complete-coverage window grows. That is deliberate.
